Localised embedded particles of Manganese (Mn) and Silicone (Si) causing material latent defect to the frame. Subsequently, the fractured frame will lose its rigidity causing the counterweight frame to derail from its guide rails.

Detachment of counterweight components can cause the lift to lose the balance, resulting the lift car to travel at high speed.
