1. Low overhead – poor provision of clearance in design
2. No warning sign – negligence of safety codes
3. Inadequate lighting – poor provision or spoiled bulbs not replaced

• Causes

1. Position makes it poorly accessible for cleaning
2. Inadequate cleaning after lubrication or other maintenance work

Travelling cable or compensation cable noisy

• Causes

1. Cables not firmly secured – careless installation or mishandling during lubrication
2. Lack of lubrication
3. Travel cable not covered with sheath

Bottom dirty with lint, cobwebs, corrosion

• Causes

1. Irregular cleaning
2. Not enough space / provision for cleaning in the lift pit

Corroded guide rail

Noisy interior

• Causes

1. Poor acoustical design unable to prevent noise from the hoistway
2. Loose false ceiling due to poor installation
3. Ventilation fan is noisy due to poor make or inadequate maintenance (e.g. damaged blades during careless cleaning)

Spoilt emergency/ trap door

• Causes

1. Lack of security provided by design. – easily operable
2. Mishandling during maintenance of other parts

Certificate plate lost/ spoilt

• Causes

1. Careless maintenance – non-adherence to safety rules
2. Vandalism
