Cause of Defects
There were inadequate number of thermal / expansion joints which resulted in the concrete being unable to take the thermal expansion during hot afternoons. Structural cracks formed and damaged the waterproofing membrane directly beneath it. This led to the ingress of water into the slab below. (See Figure)
